Orange County Dental Offices: Digitizing Patient Records Compliantly
What Orange County dental practices need to know about onsite digitization, California Dental Board retention requirements, and the practical workflow.
Orange County's dental market is dense, mature, and increasingly consolidated. Group practices and DSOs (dental service organizations) are buying up independent practices, retirement transitions are common, and EHR migrations across the region are running constantly. Each of those scenarios has a paper archive sitting in a file room that needs to move.
This post is a practical guide for OC dental administrators.
California Dental Board retention requirements
The Dental Board of California sets minimum retention at seven years from last treatment for adults, with longer periods for minors (one year past majority, minimum seven). Practices participating in Denti-Cal must retain records for ten years from date of service.
For a typical OC dental practice with mixed adult and minor patients and partial Denti-Cal participation, the practical retention floor is ten years.
This means most practices have at least ten years of paper they cannot dispose of, and probably twenty-plus years of paper they have not been brave enough to dispose of. Digitization is the path to reducing the physical footprint while preserving the legal record.
The DSO acquisition scenario
A common pattern in OC: a single-doc practice is sold to a DSO. The acquiring DSO typically standardizes practice management software across its locations (often Dentrix or Open Dental at scale). The acquired practice's paper archive needs to either:
- Be digitized and imported into the DSO's standardized software, or
- Be retained as a "legacy archive" at the acquired location.
Option 1 is operationally preferable but requires upfront work. Option 2 defers the problem but creates a long tail of one-off lookups against paper for years afterward.
The DSO acquisitions that go best are the ones where digitization happens during the transition period rather than after.
The retirement scenario
OC has a large cohort of dentists in or approaching retirement. The retirement transition follows the rules covered in our California dental chart retention post: notify patients, transfer or retain records for the retention period, and either return originals or arrange ongoing storage.
For a retiring dentist without a successor, digitization converts a years-long storage problem into a one-time project that ends with a small encrypted drive instead of a storage locker bill.
What an OC onsite project looks like
A typical onsite scanning project for an OC dental practice:
- Day 0: walk-through and quote. Look at the file room, get a count of boxes or linear feet, ask about Dentrix/Eaglesoft/Open Dental version, confirm the practice's preferred document type taxonomy.
- Days 1-2: setup, initial scanning, first QA check on the in-progress archive.
- Days 3-5 (or longer for larger archives): production scanning. Daily QA. The operator is in your designated room full-time during practice hours.
- Day N: final QA, patient matching review, delivery into Dentrix (or other system).
- Day N+1: practice staff verification, sign-off, and (if applicable) coordination of secure paper destruction.
The practice continues to operate normally throughout. Patients still get seen. Staff can still pull any record from the in-progress pile if needed.
Software-specific notes
OC dental practices typically run one of:
- Dentrix (G7 or Enterprise): major incumbent.
- Dentrix Ascend: cloud-based, growing fast.
- Eaglesoft: large installed base.
- Open Dental: growing share, especially in newer practices.
- Curve Dental: cloud-native, present in smaller practices and DSO locations.
Each has slightly different document import workflows. The scanning vendor should produce files in the format the specific system expects, with patient matching pre-resolved.
